The minimal fields
Per trade: date and session, setup name from your playbook, direction, planned entry-stop-target, actual fills, R result, rule-adherence grade, and one honest sentence of reason. The adherence grade
The single highest-value field: did this trade follow the written plan, yes, partially, no? It splits your data into two strategies, the one you designed and the one you actually trade, and the split is routinely the most expensive discovery in the book. Results by adherence grade answer whether you have a strategy problem or a behaviour problem, which have opposite cures.
The weekly ritual
Thirty minutes, same time weekly: tally R by setup name, by session, by adherence; reread the reason sentences from losers; pick exactly one process change for next week, written as a rule, not a wish. One change per week compounds; five changes at once is redesign theatre that measures nothing.
From data to rules
The journal earns its time when patterns become rules: a setup that bleeds across thirty trades gets retired; a session that never pays gets closed; an entry style with chronic partial-adherence gets simplified until compliance is realistic.
